This information allows our office to conduct a preliminary conflict check and determine whether the matter falls within the firm’s practice areas.

Submitting this form does not create an attorney-client relationship, and such a relationship will only be established if a written engagement agreement is signed by both you and the firm. Submitting this form does not prevent the firm from representing another party in the same or a related matter if no conflict of interest exists under the Pennsylvania Rules of Professional Conduct.

Please do not include confidential or highly sensitive information in this form. Information submitted is used solely to review your request and determine whether the firm may schedule a consultation.

Completion of this form does not guarantee that a consultation will be scheduled. The firm reserves the right to decline consultation requests for any lawful reason, including conflicts of interest, practice area limitations, or other professional considerations. Unless otherwise specified, consultations are limited to fifteen (15) minutes. In-person consultations are limited to thirty (30) minutes.

While you may request a specific attorney, consultations are scheduled based on availability, and the firm cannot guarantee that the requested attorney will conduct the consultation.

After reviewing your submission, a member of our staff will either:

1. Send a confirmation email with a payment link for a phone consultation between the hours of 12:00 p.m. - 8:00 p.m. on a specific date (if payment is not remitted at least 24 hours in advance of the consultation, said consultation will be cancelled);
2. Send a confirmation email with a payment link for your set time call and/or office consultation (upon receipt of your payment a staff member will call you to schedule your set time consultation); or
3. Send an email notifying you that we are unable to schedule you for a consultation.

Please provide our staff at least two (2) business days to respond to your consultation request.
